Do you care for someone who has dementia and worry that they may go missing? The Herbert Protocol is a national scheme which encourages carers, family members or friends to compile useful information which could be used in the event of a vulnerable person going missing.
Using the Herbert Protocol form, you can record all vital and current details, such as medication required, mobile numbers and places regularly visited, or previously located if they have gone missing before. Please print and complete the forms, which are available in English and Welsh on the links below, and keep them in a safe place.
